REQUIREMENTS

1.  Confirmed appointment (NOTE: Registration for National Elections in 2022 has ENDED).

2. Accomplished OFV form.

3. Valid passport (original and 1 photocopy). If your passport is not valid, it is recommended you renew your passport.
