How to apply for a Travel Document?

A Filipino citizen who does not have a valid passport, for one reason or another, and wishes to return to the Philippines, but is unable to fully comply with the requirements for the issuance of a regular passport, at the time of emergency, may apply for a travel document.

This document is valid for one direct flight to the Philippines.
The following documents are required:

 

  1. Duly accomplished Application Form for the issuance of a Travel Document – attach blank form Link here;

  2. Accomplished Affidavit or Sinumpaang salaysay

  3. Photocopy of old Philippine passport or NSO-certified Birth Certificate, or Report of Birth

  4. Travel Document fee of USD 30.00.

  5. Three (3) identical passport photos size 4.5 cm x 3.5.cm, in color, with plain, white

  6. background: Applicant must be wearing appropriate attire with collar and sleeves.

  7. Any document stating proof of emergency or urgency of travel.


Please note that the validity of travel document is only one (1) month and it is being issued due to emergency reason(s). Hence, its immediate usage is necessary.

The Consular Officer reserves the right to require additional documents from an applicant, to prove his/ her identity and/or citizenship, and ensure accurate and complete personal data entries, pursuant to the Philippine Passport Law (R.A. 8239) and the Foreign Service Act (R.A. 7157).
